NOTE: Letter to the Editor: "We were impressed with the recent article by Mader et al (ref) entitled, "Optic disc edema, globe flattening, choroidal folds, and hyperopic shifts observed in astronauts after long-duration space flight." The authors and NASA have done a tremendous job describing the possible pathological mechanisms underlying the ophthalmic changes seen in astronauts after extended space flight. We would like to offer a complementary explanation.

The authors describe cephalad fluid shifts that occur in microgravity as an explanation of elevated cerebrospinal fluid (CSF) pressure in the optic nerve subarachnoid space. The authors also cite the limited data available, which suggests that intraocular pressure (IOP) decreases over an extended period of microgravity. Both elevated CSF pressure and ocular hypotony could independently produce the changes of optic disc edema, globe flattening, choroidal folds, and hyperopic shifts seen in these astronauts. However, we would like to suggest that the combination of elevated CSF pressure and decreased IOP in prolonged space flight may be the underlying patholophysiological cause of the ocular findings described by the authors. We and other investiagators (refs) have demonstrated that the difference between IOP and CSF pressure across the lamina cribrosa may be an important contributor in the pathophysiology of glaucoma. In glaucoma, this translaminar pressure difference is created by either a high IOP or a low CSF and can slow retrograde axonal transport leading to glaucomatous changes in the optic nerve. In the case of astronauts exposed to prolonged microgravity, the opposite situation may be occurring where CSF pressure is higher than IOP leading to a reversal of the normal translaminar pressure difference resulting in a slowing of orthograde axonal transport leading to optic disc edema. The combination of increased CSF pressure in the optic nerve subarachnoid space combined with a decreased IOP may explain the observation of optic disc edema, posterior globe flattening, choroidal folds, and hyperopic shifts that contribute to changes in vision.

We applaud the continued efforts of NASA and the authors to study the causes of this outer space ocular syndrome, and encourage the authors to consider the relationship between IOP and CSF pressure in future studies."

The authors of the original article responded that interesting and valid points were raised by the letter and that additional data are needed to determine the etiology.
